2008 AOTF SCHOLARSHIPS
APPLICATION INSTRUCTIONS FOR ALL APPLICANTS

Please read these instructions before completing your scholarship application

1. The AOTF Scholarship Application consists of two parts: The Application, to be completed by the student on-line through the AOTF website, and the supporting materials to be mailed in. The supporting materials consist of two confidential reference forms to be downloaded, printed, and given to the appropriate persons for completion and official school transcripts to be provided directly from the reporting institution. Your references should mail the completed forms to Jane Huntington, Scholarship Coordinator, at AOTF (complete address appears below). All information will be held confidential. We will be requesting a Curriculum Director's Statement directly from your program director.

2. Complete and save all answers to the on-line Application by February 8, 2008 and provide all supporting data, postmarked by February 29, 2008.

3. Arrange for the completion of the personal references in sufficient time to assure that they will be postmarked by February 29, 2008. (Note: We suggest you follow-up with your references to be sure they are sent to us in time. Applicants are ineligible if references are not received.) You must create an account and sign in to the Scholarship System in order to download forms for your personal references.

4. Order all official transcripts in sufficient time to assure that they are postmarked by February 29, 2008.

BASIC INFORMATION REQUIRED

CURRICULUM DIRECTOR'S STATEMENT - The head of the Occupational Therapy Department at your school must complete the Curriculum Director's statement. When we receive your application, AOTF will request a Curriculum Director's statement from the Head of the Occupational Therapy Department at your school. The Curriculum Director may not be used as a reference.

REFERENCES (2 required) - One Confidential Reference Form should be completed by a current faculty member or faculty advisor and the other by someone of your choice from the following categories: former teacher, school official, employer, or other person who knows you well enough to assess your character, work experience, academic ability, financial need and your ability to successfully complete the proposed educational program. These references should be mailed directly to AOTF by the individual supplying the reference. Applications not containing references will be disqualified. References do not have to be typed. They must be postmarked by February 29, 2008.

TRANSCRIPTS - Official transcripts must include the Fall 2007 semester grades and be mailed in a sealed envelope from your current institution. Transcripts must be postmarked by February 29, 2008.

FINANCIAL STATEMENT - Complete disclosure is required for consideration - fill in every blank even if you must enter a zero. Please note that the figures we are requesting are for a whole year, not monthly. If you are married and/or have any dependents, please provide financial information for your whole family, not just your own income and expenses. Give as much information as possible to support your need for financial assistance.

ESSAYS - This is an opportunity to help the Committee get to know you as a person and is an important part of the application - give it some careful thought.

SIGNATURE - In lieu of a signature, by typing your name, and date, and saving the application, you are certifying that your application information is true and correct and you are giving AOTF permission to obtain a Curriculum Director's Statement.

SUMMARY

Applications must be completed on-line by midnight, February 8, 2008. Saving each section of the application with complete answers to all questions by February 8, 2008 will be considered a completed application. No changes shall be allowed to on-line applications after the deadline of February 8, 2008. Please go over your on-line application carefully prior to that time to ensure that all answers are true and up to date and then click the submit button.

Confidential References and Transcripts must be postmarked by February 29, 2008 and mailed directly to:

All applicants will be notified of the AOTF Scholarship Committee's decision by May 9, 2008. Awards may be used immediately upon receipt.
